INSTRUCTIONS FOR RECEIVING CREDITS
Participants wishing to receive continuing education credit must review the Self-Assessment Questions at the end of each section, and do all of the following:
- Sign in upon arrival at the Hepatitis Management Registration desk.
- Complete the CME/CE application provided, with signatures verifying your attendance at each session. No credit can be given without this documentation of attendance.
- Complete the Course Evaluation for each session attended and return to the Hepatitis Management Registration desk prior to departure.
Final deadline for original CME/CE application and evaluation submission: Monday, May 21, 2012.
